What NR 602 is really about

Two things make this course unlike the rest of the sequence. The first is that most of the people you see are well, so the work is surveillance rather than diagnosis: measuring, plotting, comparing against expectation, and knowing which small deviation means nothing and which one means everything. The second is that normal has a date attached. A weight, a blood pressure, a milestone or a fundal height is not normal or abnormal on its own; it is normal for twenty six weeks, or for a nine month old, and a note that reports the number without the expectation it was measured against has not made a clinical statement at all.

The third difference is who is talking. A toddler cannot give a history and a newborn gives none at all, so the account you work from arrives through a parent who is worried, tired, or guessing, and part of the graded skill is writing what was reported, by whom, and how reliably. Pregnancy adds a second patient who cannot be interviewed either. That is also why counseling carries so much weight here. Much of what changes an outcome in this course is said rather than prescribed: what to expect before the next visit, which symptom means call tonight, what to feed and when, and how to keep a car seat, a crib or a fever from becoming an emergency.

What NR 602’s assessments ask for

Weekly work in many sections alternates between the two halves of the title. A prenatal assignment wants a visit written for a specific gestational age, with the screening that belongs at that point, the findings interpreted against it, and the education that fits the weeks ahead rather than pregnancy in general. A child assignment wants growth plotted, development checked against what an infant that age should be doing, immunization status addressed, and guidance written for the caregiver who has to act on it. Discussions early in the week often take a worried parent question and ask you to answer it in plain words with evidence behind them. Clinical hours and the record a preceptor keeps of them are documentation only you can produce.

Where students lose points in NR 602

The reliable loss here is the number without its norm: a weight, a length or a pressure recorded and never compared to what that age or that week predicts, which leaves a reader unable to tell whether the visit found anything. Second is adult reasoning applied to a small body, where dosing, vital sign ranges and red flags are treated as though size and age did not change them. Third is guidance written as a list of headings rather than as advice, so the note says education provided and never says what the parent was actually told to watch for. Missing immunization status, a milestone screen mentioned but not interpreted, and a prenatal plan with no return interval attached also take points quietly.

Using a NR 602 sample the right way

Read a sample for the comparisons rather than the content. Watch how the writer states an expectation before reporting a measurement, how a parent's account is quoted and then weighed, and how the education section names what the family should do before the next visit instead of listing subjects covered. Then write your own visit, since the growth points, the dates and the family in front of you are what the whole note is built on. Should a well child sample include the parts of the visit that were normal?

Yes, and that is where the marks sit. In a well visit almost everything is normal, so the note is judged on whether the normal was checked against the right expectation and recorded in a way another clinician could compare next time. A visit that reports only what was abnormal reads as a visit that skipped most of its own purpose.

My assignment is prenatal and the example I found is pediatric.

Say which half you need and the sample is written for it. The two sides of this course share a structure, so a prenatal visit and a well child visit both move from measurement to comparison to guidance, but the expectations they measure against are completely different and a sample from the wrong side will mislead you on the details that matter.

How much anticipatory guidance is enough?

Enough that a parent leaves knowing three things to do and one thing that would make them call. Guidance graded well is specific to the age in front of you and to what happens before the next appointment. Written as a general list of safety subjects, it earns very little, because nothing in it belongs to this child.
